Permission Forms

• Request to Access Lourdes University Information – This form should only be completed if you are requesting access to Lourdes data. Approval indicates an awareness that the research is taking place. This form does not serve as a means to insure the protection of human subjects. 
• Agency Permission Form – This form shows that permission has been granted to the investigator for access to the necessary data/resources/employees to complete the study.
