Changing Personal Information

All personal information including, address, phone number and name change must be in writing, including a hand written signature. All forms must be returned via mail or fax.  You may use our change of address/name form or write a letter including the following information:

• Full Name
• Social Security Number
• New Address (and/or name)
• New Phone Number
• SIGNATURE and date

Standardized Tests & Other Services

Registration forms for standardized tests are available through the Office of the Registrar. Call 419-824-3817 or 419-824-3988  for more information.

• Exams offered and accepted by Lourdes for college credit 

Verification Forms
Loan Deferment, Insurance Forms and other verification forms can be brought to the Registrar's office or can be faxed to 419-824-3962.

To process name changes, we must have a copy of a social security card, court document, or valid driver’s or marriage license, showing the new name along with the information listed above.

Transcripts

You may request a transcript via fax, mail or in person. All forms must be returned via mail or fax.  Use our transcript request form or write a letter including the information listed below and mail or fax to us: 

• Full name, including maiden
• Social security number
• Phone number
• Complete address where transcript is to be mailed
• Whether you need an official or unoffical transcript
• SIGNATURE and date

Transcripts are processed within 24-48 hours of being requested. Transcripts are provided at no charge (unless the request is for same-day service).
