The student(s) you are taking notes for should receive their copy no later than 24 hours after class. If a student is not in class, the notes are to be provided to Christine Miller with the course information at the top, and a comment made on the contract form of the student’s absence.
Please fill out the form as completely as possible. If pertinent information is missing, payment from Lourdes could be delayed. You will receive confirmation of receipt of this form.
