Student Organization Registration Form
Please submit this form within 14 days of the first date of classes. Changes to the information listed here should be updated and submitted to firstname.lastname@example.org as they occur. This form is required for reserving any campus space, hosting activities, or application for supplemental funding. Organizations that fail to register at the start of a new academic year are considered inactive. To regain recognition of an inactive student organization, please submit a request via email to the Student Activities Office at email@example.com.
