Student Organization Registration Form

Please submit this form within 14 days prior to the end of the semester. Changes to the information listed here should be updated and submitted to activities@lourdes.edu as they occur. This form is required for reserving any campus space, hosting activities, or application for supplemental funding. Organizations that fail to register at the start of a new academic year are considered inactive. To regain recognition of an inactive student organization, please submit a request via email to the Student Activities Office at activities@lourdes.edu.
